PRINTED MATERIAL ORDER FORM

There are two ordering options: Printed Materials and Online Services.

The prices for the printed DAI materials and scoring services have remained the same for many years. The Specimen Set (a sample of all available materials) is available for $25 cash prepaid. Purchase orders are acceptable for all orders over $25.00. Add 10% for shipping and handling in the U.S.; 15% in Canada; and 20% for international orders. The cost for online services will be available soon. No credit card purchases are available for either ordering option.

* A complimentary copy of the inventory is currently available to first-time users responding to this web site who are full-time professionals responsible for student/personal development education or training in colleges, private nonprofit youth agencies, and churches. One per department please.

SCORING SERVICES

There are three types of reports which present data using percentages, means, standard deviations, and rankings. They are Institutional Report(s), Group Reports, and Individual Reports. A basic fee of $0.20 per student is charged to scan the answer sheets in addition to the fee for the various reports.

Institutional Report(s) $25.00 - This five-in-one report includes the following information:

(1) Overall Report - Development, Satisfaction, Discussion

(2) Development Report - Development items only

(3) Values Report - Satisfaction items only

(4) Advising Report - Discussion items only

(5) Dimension Profile - Dimensions only

Group Report(s) $20.00 (first time only then $10.00 per group report - Group reports are user-defined and follow the same format at the Institutional Report. Examples of group reports are: (1) Minority status, (2) Sex, (3) Class, (4) Academic Status, and (5) Special Code (local option).

Individual Report(s) $.50 per student - Reports for individual students also follow the same format as the Institutional and Group report formats.
